80% of medical equipment in developing countries is donated; 40% = nonfunctional

WHO estimates that 80 percent of the medical equipment in developing countries is donated. A 2011 studylooked at inventory lists from 16 low-income countries and showed the number of nonfunctional medical equipment in that pool is at about 40 percent.

Meanwhile, in high-income countries less than 1 percent of medical equipment is out of service.

When medical equipment breaks down in the developing world, it often stays broken. There are usually few supply chains to get replacement parts, and local technical expertise is sparse.

Even when the machinery isn't broken, it may not be useful. In some cases, the donated machines need voltage that is incompatible with the electric supply (if it's available and consistent) in the target country. Or the machine itself may work but the supplies needed to make it work may have run out. I have also noticed that there is often a mismatch between the equipment donated and the capacity of the health-care facility to actually use the machine. What good is a ventilator when you donate it to a facility that has no health care workers with training to use it or steady electricity to keep operating it?

An old but often quoted report by WHO on medical device donations states: "Only 10–30 percent of donated equipment becomes operational in developing countries." Invariably, the rest end up in hospital corridors or patient rooms or litter the outskirts of villages. Sometimes they become playthings for children.

Can anything be done to revive the broken machines — or at least make sure that new medical equipment doesn't fail as well?

A first step would be to do what you do when you want to make sure you give the right gift to someone — ask what they need.

Dar es Salaam to host the largest religious congregation in over 30 years

Ashara Mubaraka are the first 10 days of Moharram, that mark the martyrdom of Imam HusainAS, grandson of Prophet MohammedSAW, that took place fourteen centuries ago. This is a very important religious and spiritual event in the Dawoodi Bohra Calendar. Each year, Dawoodi Bohras across the world observe Ashara Mubaraka by attending religious sermons, mourning the tragedy of Karbala and remembering the supreme sacrifice of Imam HusainAS.

Imam HusainAS refused to pledge allegiance to Yazeed, a tyrannical ruler who had transgressed all that Islam stood for and had established a reign of oppression after coming to power. In the battle of Karbala, Imam HusainAS along with seventy two of his followers including his family fought bravely on the scorching sands of Karbala and acquired martyrdom. The oppressors had totally denied them food and water for 3 consecutive days and even Imam HusainAS’s six month old son was martyred in the battlefield of Karbala. Imam HusainAS made this supreme sacrifice so that truth may prevail, tyranny be exterminated and the cherished human values of righteousness, piety and virtue may ever remain supreme and uncompromised.

Following the footsteps of his predecessors, each year, 53rd al-Dai al-Mutlaq, His Holiness Dr. Syedna Mufaddal SaifuddinTUS,on receiving requests and invitations from his followers from different towns and cities, chooses a venue for delivering sermons during Ashara Mubaraka. In this Islamic year, 1438 H, the city of Dar es Salaam is blessed to have been chosen as the venue for Ashara Mubaraka, which will be from the 2nd to the 11th of October, 2016.

With the encouragement and blessings of His Holiness’ predecessors, a few Dawoodi Bohras made their homes in Tanzania more than two centuries ago. They came here principally to carry on business and trade. Finding the country a land of opportunities and experiencing the welcoming nature of the Tanzanians, they subsequently settled and made their homes in Tanzania. The Dawoodi Bohras of Tanzania and those of Dar es Salaam, in particular feel extremely privileged to host this event and expect to host about 30,000 guests from all over the world including East Africa.

During Ashara Mubaraka sermons, His Holiness Dr. Syedna Mufaddal SaifuddinTUS will narrate the tragic events of Karbala and shower pearls of wisdom upon his followers, mentoring them on the noble teachings of Islam and the principles of humanity that Imam HusainAS sacrificed his life for. During these auspicious days, His Holiness will pray for the victims of the earthquake in Kagera region. His Holiness and his followers, the Dawoodi Bohra community, offer support and help to the efforts of the Government of Tanzania in rebuilding and providing relief to the victims of the earthquake in Kagera region. It is also the intention and desire of Anjuman-e-Saifee (Dar es Salaam) to launch a scheme for the support of those affected by the calamity.

Wherever he travels, His Holiness reminds his loyal followers of the noble teachings of Islam that “love for the land of abode is an essential part of the faith” and that “religion is not just prayer and fast but also to strive for betterment in this world and hereafter”. He guides them to strive for the betterment of their neighbours and for the common good of all the people of their country. His Holiness guides and directs his followers to invest in trade and industry in Tanzania and has dedicated resources to assist them to work hand in hand with other members of the society and for the country at large. Following the guidance and directions of His Holiness, the Dawoodi Bohras residing in Tanzania continue to contribute to the growth and development of their country.

The anticipated arrival of His Holiness to Dar es Salaam is on Tuesday, 27th September at 1300 Hrs.

Kny: MSAJILI WA VYAMA VYA SIASA
26 Septemba, 2016

“Isome barua yangu…, mimi nimeamua watu waijadili umenielewa?

Re: "Babu wa Loliondo" - Medical misinformation can be difference between life and death

When I was a Peace Corps volunteer in Tanzania between 2010 and 2012, a 76-year-old man named Babu wa Loliondo from a remote Tanzanian village made international news. His name was plastered all over headlines for promising a miracle drink that he claimed was able to cure a variety of illnesses - HIV/AIDS, hypertension, diabetes, cancer, ulcers, glaucoma, you name it.

Word spread rapidly, and soon tens of thousands left their homes and scrambled to his isolated village.

People left their farmland (often their sole source of income) and stopped taking their medication. There were daily reports of bus accidents and huge traffic jams on major highways throughout Tanzania due to congestion caused by the sheer number of travelers. Several died making this pilgrimage, and the country suffered as a whole.

In a nation where 97 percent of its citizens live on less than $2 a day, many squandered the bulk of their life savings to be cured by Babu wa Loliondo, who pocketed $2,500 daily.

Eventually, the World Health Organization got involved to study the efficacy of his drug and inevitably debunked the outrageous claims. Unfortunately, despite advice from medical experts both local and abroad, many still continued making the journey until its popularity finally faded once people saw with their own eyes that there was no lifesaving drug.

I saw firsthand how medical misinformation can spread like wildfire and cause serious harm to the well-being of so many people. I argued with my local villagers endlessly, but to no avail. On a near daily basis, I was reminded of the quote: “You cannot reason someone out of something they were not reasoned into.” Only in a place like this, I thought, could unsupported medical claims be taken so seriously, despite scientific scrutiny and advice from experts that say otherwise.

Now, back in the United States, it’s apparent that medical misinformation knows no boundaries. Thanks to false claims circulating about vaccinations, measles, an extremely debilitating and often fatal disease which had hit a high of 600,000 cases a year in the 1950s but then fell to fewer than 100 in the 2000s, is now back on the rise. Outbreaks of whooping cough, polio, and other preventable diseases have also been reported all over the U.S., all due to a marked decrease in immunizations that had, at one point, nearly wiped these diseases out of existence.

I was stunned, therefore, to see a wall-to-wall exhibit in our very own New Hartford Public Library during August and September heavily promoting an anti-vaccination message. At the library entrance was a picture of a baby with the caption “I wasn’t vaccinated and I’m healthy,” along with posters alleging a link between vaccinations and autism. Although the display has been taken down, the library, a place that is supposed to be a haven for learning and knowledge, was promoting a very dangerous message that has led to an uptick in life-threatening childhood diseases.

As a medical student who has studied the history of vaccines and the fraudulent Andrew Wakefield Lancet paper that initially prompted the vaccine debate, I strongly believe that vaccines are a necessary part of a healthy society. Since their inception, vaccines have been heralded as modern-day miracles for all the countless lives they’ve saved. Now, they're increasingly demonized as threats to the very people they were designed to protect, all without any sound medical evidence.

While there is nothing wrong with patients wanting to learn more about immunizations, it’s another matter entirely when they put their children and their community at risk by refusing to vaccinate. Unfortunately, in today’s climate where we see drug prices skyrocketing overnight, a healthcare system which now places more emphasis on documentation rather than quality patient interaction, and an overall decrease in patient satisfaction, it should come as no surprise that people succumb to medical misinformation.

One of the best ways to counter this trend is through early science literacy. The ability to properly interpret scientific and medical journals will enable patients to make more informed decisions. By starting at a young age, the next generation will be capable of differentiating between medical facts and medical quacks.

Although we should encourage patients to be active participants in their own health, we also need to educate them about how to go about it safely. Science literacy in early education will be the key to ensuring future generations making better and more informed decisions about their health. If we can achieve that, then we can prevent a trip to Babu wa Loliondo.
